With the WCIRB’s move to our new Oakland office, we have updated all order forms used for requesting WCIRB products or policyholder information. Many of these forms have also been updated to include additional instructions or to update the calculation of state sales tax when applicable.

When ordering a WCIRB product or service, please use the current form. (See links below.) Failure to do so may result in the delay or return of your request.
